Patient Forms
To ensure we have the most updated information, each patient of GRC must complete the patient registration form. Complete and accurate registration forms ensures that you do not get incorrectly billed.
A Medical Records Release Form is used to request that a health care provider (physician, dentist, hospital, chiropractor, etc.) release a patient's medical records to Guam Radiology Consultants. Having your prior records allows our radiologists a better understanding of your medical history to address your individual needs.
At the GRC MRI Centers, we value your time and want to ensure that your appointment runs as smoothly and quickly as possible. To help us do this, we ask that you please complete the MRI Consent and Intake forms prior to your appointment. This will allow us to have all of your information on file and ready for your visit, saving you time and hassle.
